A LOSS OF CONTROL DUE TO THE PILOT'S FAILURE TO FOLLOW PROPER SOFT FIELD TAKEOFF PROCEDURES BY FLYING OUT OF GROUND EFFECT AT AN INADEQUATE AIRSPEED. A FACTOR WAS THE PILOT'S IMPROPER USE OF FLAPS.
